<-----Name of installer
<-----Date of install
<-----Date of the failure
Step 2:
Once Received we will review then request a proof of purchase.
Proof of purchase is what ever invoice/Receipt was provided to you by the installer.
Step 3:
Once we have proof of purchase we will set up a day for the repair :-)
